What To Bring:

• Insurance Card
• Photo ID
• Any X-Rays that you have had in the last 12 months. You can have your previous DDS office copy and email to us at smile@bowlerdds.com
• Completed forms

For your first appointment either download, fill out and bring these forms to your appointment OR arrive at least 20 minutes early to have time to fill them out before your appt time.
